Backup Cam - Two General Questions

Hi - I am installing a backup cam and have a couple of questions.

1. I know there is an access hole under the front DS door of my 06 DC. Is there another access hole near the back of the cab?

2. I need to connect a wire from my receiver so that the screen comes on when I put the truck in reverse. Any idea which wire to tap near the steering column or front dash.
