Wilwood Engineering: High Performance Disc Brakes would be the first stop to look. I know they make one for the 96-04 Tacoma, even though it's not listed. If you're concerned with braking power, go with a Big Brake Kit for the front instead. You're not going to achieve better braking by adding disc to the rear. It would cost probably more than a BBK for the front too.

I'd stay away from Wilwood.....my only experience with them has been with Honda's, but they all came with inferior rotors. I have a Baer kit on my Honda and the rotors are huge and have never had a problem. Two friends of mine have the Wilwood kit and the rotors are a LOT thinner, and warp easily.
