I'm driving a stripped regular cab with 6500 miles and 17 is the best highway mileage i've seen yet. I average 13 in the city. I try to keep as close to 2000 rpm and that gets me about 68-70 mph.
I did a trip to florida a few weeks ago. I started out with a fillup of mid-grade and went 307.9 miles, took 18.058 gal. That's 17.05 mpg and that is the best yet.

Yeah, you and me both.

Best I can get with my 05 access 4x4 is 16 to 17 highway and 10 to 12 city, US gallons. Worse in winter time. I can't imagine getting 20 to 23 highway, that would be like what, 27 mpg in our Imperial gallons, Canada.

It must be the elevation/altitude difference or something, we are pretty close to sea level here. I find it hard to believe that a Tundra can get better than the EPA rating. Mine won't come close to that and I generaly take it easy, about 65 MPH on the highway and use the cruise and keep the RPMs at 2000 or less in the city. All I can say is anyone with a Tundra that can get an honest 23 US MPG highway, HANG ON TO IT, that's like winning the lottery.
