My rear brakes don't seem to be grabbing any longer, nor does the parking brake hold as it should. My truck has 59k miles. Do I need to simply need to get the shoes adjusted, or do you think they need to be replaced? (2004 Tundra).

I'd check them, just to be safe but chances are you have some life left on them depending on your driving habits.
Give them an adjustment and see if that helps. You will need to pull the drums if you find the self adjusters are frozen/rusted.....
Drink more. It probably won't help you understand better, but it just might, and that's a chance worth taking....

Does anyone know if these are self adjusting drum brakes? I know theres a access hole in the back to turn the star wheel, but I was wondering if you could just slam the brakes in reverse to adjust them.

They do self-adjust when using the parking brake. But they are notorious for not keeping up, even if you use the PB often. Manual adjusting once or twice a year will keep the pedal up.
