I'm not questioning their veracity, but are you sure this isn't another one of those "I swear I was pushing on the brake pedal when the car just took off all by itself, and no matter how hard I pushed down on the pedal, the car just kept going faster until I hit something", kind of excuses?

This happens every day in South Florida, because of the senior population that lives down there, in such large numbers.

They're absolutely sure they were pushing on the brake, when in fact, they were pressing the gas pedal through the floorboards. lol

OH, I guess it's not that funny when somebody gets killed by these, shall we say, "confused", elderly people.

If the dealer can't find anything wrong, and you can't find anything wrong, maybe they are just confused as to where Reverse is located?

One thing I've noticed is that there is about a 1-2 sec wait before the "Ready" light comes on. If the car is on a grade and you pull it into gear before the light is on, it'll drift down grade. Put it back in Park, press the start button again and all's well.

Could this be what happened to them?

Most likely.
They simply were not instructed properly as to how to operate their car.
Whether this is a failure on behalf of the dealer, or their own, for assuming they didn't need to read the operators manual, is anybodies guess.

But one thing seems pretty clear.
It isn't the car that's broken.
It's doing exactly what it's been instructed to do, and only the operator is to fault.
