Default Intermittent low idle problem after starting

Once in a while when first starting the truck, the idle hovers around 200k RPM. During this time, the steering wheel is difficult to turn and the truck is vibrating noticably.

The idle stays this low even when putting the truck into Drive or Reverse and moving at a slow pace. It doesn't go back to normal idle speed unless I blip the throttle pretty good while in Park or get the truck up to speed.

I use on 87 octane from 76 gas stations and have tried switching to 89 octane to no avail. I tried a good fuel system cleaner which also didn't keep the problem from resurfacing. The truck has 56k miles, a new air filter, and the original plugs (which, from what I've read, are iridium).

Any ideas what the problem could be?

I'm currently running some more Chevron Techron fuel system cleaner again on this tank to see if I can remove the problem. Yesterday, the idle was abnormally low again (wasn't even on a cold start) and I blipped the throttle just a tad, which stalled the engine.

I've really got to fix this problem. If no one has any ideas what it could be, hopefully I'll be able to figure it out and post the solution here.
