Default '03 Prius Fuel Sender

On my '03 Prius I initially received trouble code P1455 "Vapor Reducing Fuel Tank System Leak Detected (Small Leak)". I reset the MIL twice. The third time it came on the fuel gauge read full then instantly dropped three bars then dropped to the last bar flashing. I fueld up but the tank was only half empty. I reset the MIL once more. It has not come on since but now my gauge always reads full. I did a fuel inclination reset procedure which didn't work. I tried to find the wires from the fuel sender at a plug in the dash but the information from ALLDATA (a repair guide from Auto Zone) was incorrect. One wire couldn't be found and the other was in the wrong pin location on the plug. I'm trying to find out if the gas tank or the combination meter needs to be replaced. Any suggestions?
