I have an '06, but here are some '04 instructions I found.
First notice that the rear wiper arm has two parts, the smaller one covers the bolt and nut. Using both hands, spread the sides of the small plastic cover and slide it off. It has two small pins that keep in place. You can then remove the whole arm as you did in the front, leaving the metal rod straight out of the glass. Please make sure to mark the arm before removing. Then it will be easy to reinstall, because there is just one right direction to reinstall the arm.
Then you need to pull out the blade with your hand or using pliers. It is a little hard, but it will move out. Your new blade should be 15" long. Reinstall the blade slowly through the arm handles. Once it is completely installed it will fit inside the arm. Reinstall the arm back on the metal rod. Click it in place and then install the small plastic cover with pins by expanding the slides and snapping it in place.

I realize this is an old post, but I would like to answer with what I did, as this forum comes up when you search google. And, I am sure people are still struggling with this ridiculous design.
First
There is little chance you will find an after-market refill for this wiper. It is considerably narrower where the rubber connects to the wiper. It is apparently available somewhere, or others have forced the wider aftermarket refills to fit. I tried and was unable. So I ended up going to the dealer....
Second
If you buy the replacement from the dealer, they give you the whole blade (but not the arm)
It is very difficult to get the old one off due to the fact that the arm will only lift about 2 inches off the glass. This prevents removal of the blade.
So here is what you do:
As in the previous post, remove the plastic cover at the base of the arm. It is just a press fit, but it is snug. You need to pull the 2 sides away from the center, and while apart, simultaneously pull the cover up away from the glass.
NOTE: here is where this is different from previous post.
You do not need to remove the arm. With the plastic cover off, the arm will lift completely like any other. There is no normal squeeze-together clip like on normal arms. You simply rotate the blade tip away from the glass (not quite 90 degrees), then you can pull it off. It makes a clicking sound when you rotate it far enough.
Thanks Jon. I was about to bring the vehicle to the dealer where i bought the replacement blade, as i couldn;t take off the old one - & i almost ripped the whole thing off, arm and all. Glad I saw your post - it was too easy.
