I took the van to the dealer today to get the door fixed. A black wire is broken, and the dealer claims that due to a broken cable on power door system and motor will need power door motor assy (about 1800 dollars)
I have Extended warranty to 72 Months/200,000K but I am told that this repair is not covered.

I would read the extended warranty with a fine tooth comb or have the dealer show you specifically in the contract where this part is not covered. Sounds like BS to me.
The dealer is very supportive. I have read through the ECP document and I do not see the power door motor, but I also dont see where it is not covered. Bottom line, the cable is broken, but apparently cheaper to replace the entire unit (motor and cable) vs fix the cable. I agree it is BS and if others have had a similar situation, or can offer some suggestions, I am all ears

We'll speaking from one that has change SEVERAL of these. If you havent done it before its a pain in the butt. I have done so many that I can do them in about 30 minutes. You can do it with or without the door on the vehicle. Personally I do them with the door off. You can't fix the cable if its broke. The cable has to slide down 2 sets of sliders and in and out of the motor theres no tolerance for a splice anywheres. I had no idea those motor assy were that expensive.
The way this is changed is you remove the door. (disconnect negative on battery, or turn power slide door button to off) Remove the panel, drop the glass down about a 1/3 the way, pull the 2 rubber grommets off from inside the mod. Remove 2 10 mm bolts, remove the back sash on the door and the inside rear window rail, pull the inside rubber window run slightly out. Slide the old glass out, drop the mod down, remove the cable assy bolts (4 10mm bolts) , unplug the wiring harness, remove the 2 10mm bolts atop the rear slide roller, theres also one 10mm inside the door at the very top near the rear slider. Remove that bolt too, its grey and about 1 1/4" long. You also have the remove the rear latch lock, 3 T30 torks bolts. Unsnap the cable from the 2 retaining clips and slide the power door assembly out the bottom of the door. Reverse assembly for the new one. Id imagine they are charging you about $300-400 labor. Id do it for $100 lol No idea where to get a new one reasonably tho. Its sad cause I see 1000's of them daily. Ask me how I know this? I work in offline repair at Toyota Motor manufacturing Indiana.

I'd take it somewhere else. You can get a power door assembly from a used parts dealer for a few hundred dollars. With a power jack and about four hours of your time, I've heard you can fix it yourself.
Dealers are generally a rip-off from my own experience.
There's a sequence for that motor if you replace it yourself you could possibly break it again I've seen those motors split easily just by letting too much cable go into the motor assembly the spool that holds the cable can only hold just the right amount to open and close the door

I agree, if you don't know what your doing with the new motor your going to shell it out quicker then you put it on. This is where the dealer comes in handy. I've replaced a few and from I've seen they usually run around $1100-1200 without labor
I have had it with the 2004 Toyota Sienna power sliding door.
Fortunately I only have one on mine that has the power feature.
A couple of weeks ago I had a door ajar light I pressed the button for the the right passenger sliding powered door. I pressed the button to open the power door and my passenger attempted to manually close the power door as it was opening and the power door seized half way open. I got out of the vehicle and attempted to release the door from the outside. The power door motor appeared to continue to attempt to open the door even though it was seized. I attempted to assist the opening by applying some light pressure however at that point the door derailed from the lower slider rail which caused the right rear window to shatter. The vehicle was unusable at that point and I drove it back to my garage and disconnected the battery since the power door was still attempting to open which was causing some minor body and paint damage. The following morning I drove the vehicle to Mayfield Toyota for an estimate and repair. I was given an estimate of $3300.00 from Mayfield Toyota in Edmonton and that it would take approximately 1 week to repair. $1800.00 was for the power door motor. I was asked if I had extended warranty but I see in this forum it probably would not have covered the motor. It was all repaired at my cost. Today however (1 week later) the power door again stopped half way
and seized. It would not close manually either even after the power to the door was disabled. I called AMA since we were basically SOL again on a Sunday before Christmas eve. The only solution was to cut the cable to close the door manually. The vehicle will go back to the shop and I am going to request a full refund and have all power functionality removed from this door. It's a liability and a complete waste of time and money.
