I have an issue with my 1995 4runner SR5 2WD. The Transmission seems to work perfectly fine with what ware and tare it has from the 161kmiles on it. But! sometimes when the vehicle comes to a complete stop then accelerated it seems like its stuck in 3rd or 2nd gear...the engine will rev up but laggs so much you have to physically shift the gears to drive it.
when its accelerated it is able to downshift into a higher gear then back to a lower one as the RPM comes close to redline. But at a dead stop it will not shift into 1st.

This only happens occasionally though. At a time it would only shift from 1-2-3 smoothly in the morning, at the first stop sign it would stop working correctly and wouldn't automatically shift into 1st gear.
