Taking it out is pretty easy. Lower the tail gate, you will see two bolts that go through the black metal D pillar and into the taillight assembly. Remove those, one is towards the top, one towards the bottom. The bolt inbetween with the washer doesn't need to be removed. Then you pull the whole shabang straight out. The outer side of the light assembly is held in by plastic rivets. The kind with a ball and socket. It might seem a bit stiff and hard to come out, just be carefull and get your fingers in there and pull back.
I just installed the bed extender and you need to take the rear light assys out to mount the brackets for the extender. If you can find the instructions for mounting the bed extender, you'll see the removal procedure in there. Check any of the Toyota part online retailers, most have pdfs for the installs.
