Default Transmission Issue

Sometimes when shifting into 5th gear from 4th, accelerating to highway speeds (60 mph), my transmission will make a scratching/gnashing noise that I can also feel through the shift lever. The clutch pedal is on the floor and I don't bang gears around. My instinct as a mechanic tells me my oil is definitely worn out, but is there any damage to the guts of the trans? I have a 2005 tundra with the 1gr engine and RA60 trans, with 35000 miles on the truck. Thanks in advance!
